Driveway repair

My driveway has a Crack which has now raised 3 inches at one end. I'd like that section of the driveway leveled, i.e. concrete lifted and re-laid. There is a tree beside it whose roots may be contributing but the main contributor was cabling along the front of the property installed using high water pressure which lifted the driveway a few years ago. And recent earthquakes haven't helped.
