Crack through concrete pad of sleepout

Carpet has been lifted in sleepout revealing a crack through the pad. It was not seen in the EQC assessment (was under carpet) and I need someone to tell me. 1. If this was likely to be earthquake-related 2. How much to repair (in any case). There is also a gouge out of the edge of the concrete under the metal sliding door, which has penetrated right through to outside. I need to know if I can just fill this with a gap filler product, or if something more drastic needs to be done. Picture attached. Probably about 1 - 1.5 m in length
