Replace wooden door

We need a door replaced and installed. Its a wooden door the runs from the house onto a deck. It's 1980mm high x 600mm wide x 40mm deep with two windows. We need the current door removed, the new door made, the lock removed from the old door and replaced in the new door, and the two windows moved into the new door. We then need the new door hung.
