Sealing concrete path

The concrete path to our front door is leaking water down to the garage beneath when it rains heavily. The concrete seems to have some small gaps where it is joined to the house and has been patched up in the past. It also seems slightly porous in general. I'm looking to get this sealed up as soon as possible. Cheers, Frank
