Concreting of existing driveway

Concrete driveway - area approx. 40m2, existing driveway appears to be gravel or base course with some dry cement thrown on overtop and wet down. Appears to be boxing on one side. Driveway on slope, not sure of degree angle maybe around 15deg?? Looking for budgetary figures so we can start putting aside money towards it, doesn't need to be a decorative finish just plain concrete to match existing driveway best as possible. I can assist with labour if this keeps costs down. May need to be excavated first? Would prefer someone who can offer written guarantee of their work.
