Repair around manhole

We need to tidy up the concrete and paving around an existing manhole. Please see the photo attached. For the area marked in blue, we like the old cracked concrete to be removed and the two pavers to be re-positioned and re-concrete with strong concrete. There is a small ridge that need to be filled (shown).
