Insurance condition report for 43 ft launch

I have a 1987 Princess Redeemer Launch which is up for its annual insurance renewal and due to the age of the vessell the insurer is asking me to complete a condition report which must be prepared by anyone who habitually builds, works on or surveys boats. Can anyone help? Boat is based in Westhaven
