Install a phone jack outlet

We currently have 4 phone jack outlets in our house. One of them doesn't work. However, we only need one jack (we use a VoIP phone system) and it has to be in our study near our modem. Need someone to supply and install a phone jack outlet in our study. We are available for the install after business hours and Wednesday afternoons. Would appreciate a quote/estimate incl GST when making contact.
