Need a COC for existing separate electricity cable of hot water cylinder

I want to apply for a controlled tariff meter which require a separate electricity cable of hot water cylinder. This job has already been done hard to trace back for a COC, but WELS staff came and asked for a COC. without COC he could not continue the meter job. Therefore, here I post a job for an licensed electrician came to check to offer a COC and quote (if need to adjust, please also help to quote), and also most importantly, when WELS staff came again, need this licensed electrician came to meet and assist him. Thanks.
