Replace bathroom extractor fan

Need through-wall bathroom fan replaced. Bathroom fan is located above shower. Complicating factor is that the fan is only accessible from the outside, which is a service duct in our apartment building. The service duct is easily accessible and has a built-in ladder. However the fan is three floors up so a safety harness would be prudent. Need a gravity grille on the exterior or other method to prevent backdraft. Fan appears to be 150mm type.
