Pathway removal

We have a pathway running across our lawn approx 12 metres in length ,1 metre wide and ~3 inches thick. Goal is to get is totally gone and converted to lawn. Wanting cost breakdowns for the following: - Breakdown of the concrete (and just left in chunks pretty much as is) - Moving concrete chunks to a skip - Alternatively, putting concrete straight onto truck for removal if possible. Reason being is that I don't know if I've got the time to sledgehammer this myself, but I can possible haul it into a rental skip myself, and just need someone to come use a jackhammer and make it quick work of it. But if the disposal can be arranged for a good price too and save me the skip rental (plus needing to buy a wheelbarrow) then that would be appreciated. No real timeframe but depending on cost, can be done soon.
