Concrete Slab needed

I have attached pictures .. I want this area to be concreted. My partner has brain tumors with more surgery ahead .. the area is our front entrance and is uneven .. slippery and dangerous. I m happy to lift all bricks pavers and stone myself... i can use them else where. Area is 6.5 mtres x 4 ... roughly , I need a quote and a specific day that the job is booked in for and that it will be done. Not interested in being mucked about it has to be done on time
