Concrete slab for spa

I'd like to install a spa, and I need an approx 2.5m^2 x 100mm pad sufficient to hold the weight of the spa. Looking for quotes this week, would love to have it done soon. Access passes under a carport and up some steps. Driveway is ~2.5m wide. Not ideal for a huge concrete truck, but it's near the road and a neighboring drive that runs behind.
