Ramp from driveway to new garage

I have a new garage adjacent to an existing concrete driveway/car pad. I need the concrete access ramp boxed and poured between the concrete pad & the garage door. The garage door is 3m wide, the garage is 4.8m wide. The gap between the pad & garage is 1150mm, the pad is 100mm thick, the rise from the pad to the garage floor is about 140mm (12% slope, steepish but acceptable). Sometime early May would be ideal. I'd like the ramp sloping up to the door, but each side of the door sloping down to the sides so it is horizontal between the pad & garage at the outside.
