repair gap between house and yard

Around the back of my house there is a gap between the house and a concrete verge. the gap is about 1-2cm wide and runs about the length of the house. During heavy rain water is getting in the gap and inside the walls of the house. It appears that originally there was a rubber seal that has eroded away. I would like the gap to be fixed so that no more water gets in the side of the wall and to investigate if any extra work is required around the area to bring the wall up to a good standard.
