Replace house foundation vent & minor concrete repairs

I have the following small repair jobs: 1. Source and install a masonry vent (approx. 30cm x 14cm) in house foundations. See pic 1 where I've blocked the hole to keep animals out. Pic 2 shows a good vent near the broken one as an example of what's required. You'll also need patch up around it with the rough finish concrete. - Repair chipped concrete on porch where old timber structure has just been replaced. See pics 3 & 4. - Refix a couple of loose blocks on wall. See pic 5.
