Repairs to grave and add a plaque

Hi, I'm enquiring about the cost of repairs to my family's grave in South Malvern cemetery. Location is about 4 headstones in from pedestrian gate. See photos. Also cost of adding a small plaque to include the names of their children who were not buried anywhere.
