Licensed Building Practitioner for EQC report

Hi there, we own a 2 story rental in St Martins which had been repaired following minor damage from the earthquakes. We have noticed cracks reappearing on some walls and the ceiling, getting more noticeable over time, and cupboards that are now jamming and difficult to open, which we think probably the filling of cracks and paint has not sorted the earthquake damage. We have been in contact with EQC and they have requested a report from a LBP (a Contractor Statement of Damage online lodgement form). We would like someone who is familiar with EQC repair work and their review process to assess the situation and submit this form. This is the first step, with of course the actual repair work potentially following if everything goes to plan.
