Leak under Kitchen Sink - Report for Insurance

I found a leak under the sink in my kitchen on Friday 19th Aug. I believe I fixed the problem (pvc fitting was not tightened) but as I'm not qualified, I'd love for someone to come out and assess, and provide a report for my insurance this week (ending 26 Aug). It has damaged the MDF on my cabinetry and not sure if there's further damage underneath. The house is less than 2 years old.
