Leaking kitchen tap and dripping pipe outside

The tap is leaking at the base where it joins the sink. It doesn’t leak if the tap is at a certain angle but as soon as I move it it leaks. The pipe that is dripping is directly behind the toilet and drips into the garden outside. It drips constantly.
