Concrete slab subsidence

I have a house with a concrete slab that appears to be affected by ground subsidence. There are extensive cracks visible in the garage. I believe there are cracks elsewhere but this is hidden by flooring. There are areas in the house where the flooring is not level as well as cracks on window and door frames. Externally there are cracks in the weatherboards. I think I’m looking for someone to: 1. Identify, if possible, the root cause of the subsidence. 2. Recommend solution(s) for addressing the root cause, if possible. 3. Recommend solution(s) for re-levelling the slab and strengthening the foundations. 4. Implement the solutions. Please be clear if you can help with some, but not all, of this.
