Concrete Slab Box and Pour

I’m after a guy to box a simple 13 x 18 rectangle slab and supply and pour concrete. We can level it. To be done around September-October 2019. Would like the the perimeter (30cm) to be at least 20mm thick but the rest around 10-15mm thick. Cheers.
