Drive crossing repair

The concrete vehicle crossing at the top of our driveway is cracked above some utilities - I think water, power and telephone. We need a panel replaced as per the attached photo to get our bond refunded by the council after our renovation. The job is to repair the crossing to the standard required by Auckland Council.
