Concrete Pad for Shed

I need a concrete pad for a garden shed that measures 4.25 x 2.57. I want the pad to sit about 100 mm above ground. Access to the location is very good, all equipment can be plonked right next to the job. The shed will sit close to the fence and the woodshed on the left (Pictured).
