Concrete foot path repair

Have concrete footpath down one side of the house approximately 25 meters in length, this leads to concrete patio that’s 14 meters in length. The concrete footpath has come away from the house 25-30mm along the length of the house, the concrete patio has also cracked at the cuts and opened up 10-15mm. Is it possible to inject mud/foam polymer under concrete and see if concrete would move back into position or just best to fill these gaps in with concrete? House is on edge of a hill and this hill did have pine trees on it, few years ago these got cut down and that’s when things moved, engineers report put it down to shrink/swell of the ground.
