Raised concrete patio with form work

We are considering adding a porch off the lounge and extending it to an existing porch. I would like it to match the existing porch which is concrete. The new porch would be about 7 metres long by 3 metres wide and involve some curves. The design is not final and I am after so costs so I can decide whether to go with this design or a larger composite deck design. Another option is to just do the form work for the walls and steps.
