Cut concrete drive , dig, re attach broken conduit, reinstate concrete drive

Hi i recently had chorus technicians try and install fibre, however they damaged the under ground conduit (the green pipe has been yanked out). I need to have a small area 50cm x 50cm of my driveway cut. Then dug out to access the conduit and reattach. I then need the cut area reinstated to look as close to what was originally there.
