New back path

The path along the back of our house was removed so that drains could be replaced. I need a new concrete path put down. The area is roughly 1.5 metres wide by 20 metres long and we need the concrete to be around 15cm deep. It needs to run off on the length furthest from the house into a contrete drain ditch that already exists.
