Concrete repair for back door entrance

Entrance to the back door has concrete path leading to a concrete slab as the floor. There is a small wooden porch that is currently bolted to the existing concrete slab but because of movement one corner has started to crack. We just want to repair the concrete and reattach the wood porch post to the concrete floor again using steel plates/brackets.
