Leather Sofa Cushions re-foam

I have an old but very well made brown Italian leather sofa. The seat cushions are now sagging due to age so I'm wondering if they can be re-foamed. One of the cushions has a tear which started out as cat scratch a few years back. Not sure if that can be strengthened or repaired. I'm not interested in a full leather replacement at the moment as we still have cats and dogs. The main aim is to restore its comfort.
