Need ramp build and added to backdoor entrance of home

I need to have a ramp with railing at my back door entrance built. I will be undergoing hip replacement surgery Feb 17th and will need a ramp with railing installed if possible. The steps in height total is 14 inches. The top step is approx 86 inches wide and the bottom step is about 110inches wide. I hope the images help
