Range Hood Light and Fan Switch

The slider switch for the range hood fan and lights is cracked and sometimes works/sometimes not. We need someone to replace this (i think). Also, for one of the lights, the 'clamp' that holds it in place doesn't work... it has been superglued temporarily superglued in-place so would like to get this sorted as well.
