Damp investigation and repair

We have a small patch of mould appear on the carpet of a room adjacent to bathroom. Inspecting under that bath the timber attached to the concrete floor is blackened with mould in the corner between the bathroom and affected room. In first instance need to asses the root cause. Is it a leak or rising damp? Repair will then be required.
