Replace upstairs (main) bathroom with staircase (and small toilet if possible) to allow internal access to downstairs. Dig out a bit of earth under house to fit new downstairs bathroom. Lower downstairs floor (possibly dig out more earth) in order to increase ceiling height, currently downstairs is not tall enough to be considered living space. Maybe retrofit underfloor heating to upstairs if economical. Require a builder/project manager, someone with enough experience to foresee major challenges and give a quote with some confidence. Unsure whether an engineer is needed or whether the job is even practical/achievable given limited space. House is roughly 1920's, wood frame, potential borer issues will be uncovered. No strict time frame, probably wouldn't want to start destruction/construction until late winter/early spring.
