We had simultaneous discharge from the inlet control valve and cylinder relief during use. There’s also been long-term dripping from the relief valve.
Replace the inlet control/PRV setup with a proper mains-pressure cylinder valve set, replace the hot cylinder relief valve, pressure-test the incoming mains, confirm the correct kPa rating from the cylinder label, and PRV discharge outlet safely outside where visible
