Broken Tee Pipe - external

I have a Tee-Pipe in my driveway that is broken - it is under the driveway and next to a drain area. Drain area has constant water gushing into it, and area around drain is now always a muddy puddle. I had the same problem 18 months ago (which is why I know exactly what it is). Need to drill a small hole in the driveway, replace the Tee Pipe that goes from the main pipe to my home plumbing (house is up a driveway with multiple houses) and then replace the seal/concrete (whatever it is) within the driveway. The picture attached shows the size they needed last year (black area is what they drilled in, fixed and replaced) Preference would be as soon as possible.
