Water leaking from fire place’s roof

Rain water come inside to house from fire place’s roof. Need to fix properly. It might be minor fix. Need to repair plate & fix properly to ceiling & check it from roof. Nothing was leaking from ceiling void/ under roof. Water was leaked direct to the this ceiling plate shown on attached pic.
