Garage door motor repair/install

We need the garage door motor that came already in place when we purchased the house to be fixed or replaced. We don't have any remotes for it, so ideally if we can purchase remotes for it also. I have attached photos of the model and out garage set up. Please provide a quote for parts and labour, and any timeframes. Further, expecting that this motor is likely a dud, the cost of a new motor install. We don't have a critical timeframe for this install.
