concrete floor re-levelling

Hi, I need to relevel the concrete floor in my room. room size: 2.3m x 4m It is carpeted at the moment. It is not a earthquake damage, but for some reason it was built like that. It is slanting from the center of the house to the outer walls. About 12mm dip. Thanks Jas.
