Driveway repair

We have an existing driveway that has cracked and is subsiding. This has caused damage to an adjoining pathway and house utilities (power and water). Need the damaged driveway and pathway removed and replaced. Would also like a grate cut into existing concrete to channel water away from front door during winter. Location is down a long shared driveway which is steep in places. Work is on a private driveway which connects to the shared driveway.
