Concrete slab

I need a rectangular slab of concrete 7m x 5m at the bottom of a wooden ramp which is the house entrance to make access to the car parking area for a person who has poor mobility - wheelchair/ walking frame. Currently not easy to push disability equipment on stony path/driveway area. It will need to be reinforced in case an ambulance parks on it! Otherwise only used for cars. My son has an excavator and will prepare the site and lay sand. It just needs the wooden round the edges and the concrete poured and smooothed..... Like it done within a month if possible
