Concrete prep and pour - driveway and side pathways for new build

We need quotes on prepping and pouring a driveway and parking area, 1m concrete pathways around the front and side of the house, installing/connecting a slot drain in front of the garage, reestablish concrete road crossing etc as per a Council consented drainage plan which specifies: "...A road surface comprising 150mm thick concrete driveway reinforced with one layer of 665 reinforcing mesh has been recommended for the new driveway. A 100mm thick compacted AP65 pavement on compacted subgrade - Min. CBR 6 - is to be constructed to support this driveway..." There is a small piece of old concrete to remove and the contractor will need to work around the levels of the sump and soak pit for the driveway storm water design. 1. To complete dig out and remove to cleanfill. 2. Complete all preparation with 40mm basecourse all compacted. 3. To complete all boxing to correct height. 4. Supply and lay all steel mesh 665. 5. To supply and place all plain concrete which will be 13 mm structural 20mpa cement base all concrete will be 150mm thick. 6. To complete the surface with a slate finish. 7. Complete all saw cuts 2-3 days after finished concrete work. 8. Concrete pump included. 9. To supply and set up slot drain and sump which will be connected to outlet for soak pit. 10. To complete 2 x side footpaths 100mm thick 1mtr wide. Work to be done in the next 4-7 weeks (once drainage work is done) - only quote if you can a deliver cost effective, high quality job.
