Driveway replacement

I have an old asphalt driveway that is falling apart and needs to be replaced. I am interested in quotes to rip it up, rebuild and level out the base layer and relay a good quality asphalt or alternatively redo it all in concrete. The driveway is about 17 metres by 3.5 metres and the area outside the garage is 7 metres by 9 metres. Total area to do is about 124 sq metres. There are a couple of council man covers on the driveway, see the photos for where they are.
