Concrete parking pad starting to crack and slump

My concrete driveway parking pad has developed large cracks across the surface over about 5m square. One slab of concrete (5m x 5m) is now 2 cm below the adjacent slab. As well this slab has slumped in one section and is causing a hollow depression to appear. The cracks appeared a year or more ago but in the last 2 months has become much worse. The concrete pad has developed a gap underneath it clearly visible.
