Concrete Slab for Pizza Oven/Table Top

Looking for someone to build a concrete slab that can be used as the base for a Pizza oven and also a bench top area for prep of pizzas etc. The idea is that there will be a Pizza Oven built (from a kitset) on the left hand side. Then prep area in the middle and a BBQ will be slottted in on the right had side. Refer to the attachments for more info. Looking for the top of the slab to be smooth - as it will be a food prep area for family pizza creations.
