Concrete lift/leveling which has sunk due to soil

There are two areas to be addressed. *green rectangle area - when it rains the area floods and is likely because concrete has sunken. Need to probably lift concrete up from the soil. Approx 9m in length. *pink rectangle area - concrete sinking so needs to be lifted up so it's level as well. Approx 210mm x 50mm. Happy to review any ideas on a fix please. Thank you.
